Manatee County students to recieve H1N1 vaccine consent forms the end of October

BRADENTON, FL-- Officials at the Manatee County School District say they will not distribute parental consent forms for voluntary swine flu vaccinations until the end of October.

The school district is partnering with the Manatee County Health Department to provide swine flu vaccinations to students in elementary, middle and high schools.

Health Department workers will give the vaccinations to students whose parents or guardians have provided consent forms.

The forms will be distributed inside information packets.

Parents who want their children to receive the vaccine must have their child return the completed form to their school within three days.
