Alfonso Eugene Mackey, 31, was arrested upon his release from Bayfront Medical Center, and charged with DUI manslaughter, vehicular homicide, second-degree murder, and aggravated fleeing and eluding.
An arrest affidavit indicated that Mackey's medical blood tests screened positive for cocaine, marijuana and a small amount of alcohol.

The armed robbery occurred at the Family Dollar Store at 1720 16th Street North around 8 p.m. on March 21.
Police say Mackey and the second suspect demanded cash from the clerk. Mackey, according to police, was armed with a rifle.
The men demanded money, police say, but the clerk was unable to open the register. The men ended up fleeing with packs of cigarettes.
Police say the men took off in a Monte Carlo parked behind the store. Mackey was believed to be behind the wheel and led police on a chase that ended in a fatal crash on 15th Avenue South.
Mackey was found behind the wheel but his male accomplice was not in the car. However, a 19-year-old woman was in the passenger seat. She died at the scene and has been identified as Dijon Jackson.
"When the surveillance team was following the vehicle they saw it stop briefly," explained Bill Proffitt, spokesperson for the St. Petersburg Police Department. "One of the suspects may have gotten out the car. We don't know if the woman got in the car then or if she was already in the car."
Minutes later, officers spotted the suspect vehicle, and attempted a traffic stop.
The vehicle sped away, and a supervisor authorized a pursuit.
Moments later, the suspect vehicle crashed into a tree at 15th Avenue South and 40th Street South.
The suspect police are searching for is described as a man in his 20s or 30s, with dark complexion, who was last seen wearing a black jacket with red around the collar and down the front of the jacket, blue jeans, and a black New York Yankees cap. During the robbery, his face was covered with something black.
